Preparing For Your Visit
To make sure that your visit goes smoothly, we recommend arriving 10 to 15 minutes before your scheduled appointment so we can help you get started on necessary paperwork. Our team will assist you with any questions you may have to ensure your check-in process is efficient and stress-free.
Be sure to bring a valid photo ID with you to your appointment. We accept the following forms of identification:

Citizenship certificate with photo and full name
If you are unsure whether your form of identification is acceptable, please call before your visit. We are happy to help!

Kaizen Clinical Partners accepts a variety of private and commercial health insurance plans, including public options like Medicare and Medicaid. We also welcome self-pay patients, meaning those without insurance who choose to pay for their medical services out-of-pocket.
If you are using insurance, please bring your insurance card to your appointment. The name on your card must match the name on the form of identification you provide. If you do not have your card or if it is expired, let us know. We can help you connect with your insurance provider to ensure proper coverage and elimination of unnecessary out-of-pocket expenses.
Be ready to pay any copays, deductibles, or balances due at the time of your visit, whether it is an in-person appointment or a virtual telemedicine session. Debit or credit cards (Visa, MasterCard, etc.) are accepted forms of payment.
Please note: We are unable to accept cash at our clinics.
Please bring all the medications that you are currently taking, or a list of them, including name, dosage, and date prescribed. Please include over-the-counter drugs and supplements. Knowing this information helps our care team get a complete picture of your existing conditions, treatment plans, and medication protocols.

What To Expect After Your Visit
Depending on the reason for your visit, you may need lab results, medication refills, specialist referrals, or a follow-up appointment. If any of these next steps were discussed during your appointment, rest assured that our team will try to arrange them before you even leave our clinic. Should a longer wait time be required, we will contact you directly.
Estimated Turnaround
Depending on the test, the wait time for lab results varies and may take up to 4 business days with specialized testing taking longer. Urgent lab results are mostly available within 24 hours. If there are extenuating circumstances and we do not receive your results in a timely manner, we will update you.
Referrals to specialists and healthcare facilities are completed the same day or
within 3-5 business days. If your insurance requires prior authorization, the wait time for a referral can take up to 7 days or longer depending on the complexity of the care or specialty. Urgent referrals are processed within 24 hours.
Medication refills are issued to your pharmacy within 24-48 hours. Refills requiring prior authorization may take up to 72 hours.
